This is an approach that utilizes existing, trusted community institutions, people and organizations located within the community to affect positive change. These people and organizations can bridge the gap between people in need of help and larger, outside organizations with resources.

These leaders and individuals some of whom are in recovery themselves provide trusted education, accurate information, and support (including addressing social determinants), in an efficient and timely manner to connect people in need to the care necessary for their success and well-being.
